ARE YOUR SERVICES COVERED BY MY INSURANCE?

We take Medicare and all major PPO insurances. We charge an annual fee plus bill insurance for visits. This annual fee is FSA/HSA eligible. By doing this, we keep our concierge fee reasonable. To stay in network with your insurance, we must collect copays and deductibles. Please call our office to see if we are in-network with your insurance. We do also see cash-only patients on a case by case basis including visiting international family members of our concierge patients. Unfortunately, we are not contracted with Medi-Cal.

DO I HAVE TO PAY THE YEARLY FEE TO BELONG TO YOUR PRACTICE?

Yes. This annual fee covers everything insurance doesn't and allows us the opportunity to have longer appointments, same-day/next-day appointments, coordination of care and direct cell phone access to your physician. We have structured our practice so that your fee is FSA/HSA eligible. Please always check with your accountant as this can change anytime laws change.

WILL YOU DO HOME VISITS?

We always encourage you to come to the office but if you can't make it, we offer home visits depending on location for our senior patients. For our standard fee, up to 4 home visits/year will be included depending on location. If you would like home visits only, please call for more information. 

DO YOU HAVE A PLAN FOR FAMILIES?

We only see adults but with the paid membership of at least one parent, adult dependents aged 18-26 on the parent's insurance with the same permanent address can be part of the practice with reduced fees. Please call for more information.

WILL YOU COME TO THE HOSPITAL?

At Mills Peninsula Medical Center you will be the admitted and cared for by their excellent hospitalist team. We communicate and coordinate with your treating team and specialists. We are on the same electronic health record as all Sutter hospitals/PAMF/Stanford/UCSF in case you are hospitalized or see specialists there and they will have access to our records as well.

WHAT ARE SOME ADDITIONAL SERVICES ?

We currently offer Myriad genetic testing to qualified patients, CardioIQ testing, case management assistance and common vaccines. Cooking classes and live lectures are currently on hold during COVID. We will continue to add helpful services and welcome suggestions from patients.

DO YOU MANAGE CONDITIONS REQUIRING CONTROLLED SUBSTANCES?

We are not accepting new patients on chronic controlled substances except for stable ADHD with record of previous evaluation. Acute pain/anxiety/short term  sleep disorders will be managed as per current guidelines. If you are interested in tapering off sleep and anxiety medications, we can assist you. Please call to discuss.

WHAT IS YOUR NEW PATIENT PROCEDURE?

Please call our office to schedule a complimentary Meet-and-Greet with Dr Selvakumar. This visit is meant for you to meet Dr Selvakumar and see if we can meet your healthcare needs before committing to a year-long contract. No medical advice is given at these meetings. If you decide to go forward with our practice, we can quickly onboard you into our practice.

CAN OUR COMPANY OFFER THIS TO OUR EMPLOYEES?

We welcome start-ups/small companies/executives interested in the convenience of same day/lunch time/televisit, coordinated care and prevention. We conveniently have laboratory and radiology in the same building and are centrally located in downtown San Mateo across from Mills Health Center (Sutter). Our model can save employees and companies money on their healthcare overall, especially those with high deductible plans. We prevent problems before they start, avoid unnecessary testing and specialist referrals and avoid polypharmacy. For executives, for less than the cost of an annual "executive physical", you can have a year of evidence-based concierge care.
